We are working with Health Canada to create a reporting tool for parents and allergists to report reactions to emerging allergens in children up to the age of 17 years (at time of reaction) that occured within the last 12 months, to foods that were made or purchased in Canada.

This is designed to be a parent-led process, and we encourage allergists to complete the survey DURING the patient visit to ensure the most accurate information is collected.

"Emerging allergen" is defined as any allergen outside Health Canada's top 12 priority allergen list:

Eggs, Milk, Mustard, Peanuts, Crustaceans, Molluscs, Fish, Sesame, Soy, Sulphites, Tree Nuts, and Wheat and Triticale

This is a pilot to test the usability of this tool. After the pilot is over, we will look at the data and make improvements to the tool. Thank you for reporting your emerging allergen reaction.

To begin, we need to know whether you are a parent/guardian or an allergist.

You have selected an option that triggers this survey to end right now. To save your responses and end the survey, click the button below to do so. If you have selected the wrong option by accident and do not wish to leave the survey, you may click the other button below to continue, which will also remove the value of the option you just selected to allow you to enter it again and continue the survey.
The response has now been removed for the last question for which you selected a value. You may now enter a new response for that question and continue the survey.
